Player Medical Release - Each player on the team must have it filled out and notarized
Player Membership Form - Each player on the team must have it and signed by parent and player
Volunteer Kid Safe Form - Any adult associated or interacting with the players must fill out this form
Player Add Form - Any player added after a state roster has been completed must fill out a player add form
Player Release Form - Any player released after a state roster has been completed must fill out a player release form
Coach Membership Form - Each coach on the team must have it filled out and signed

Team Fundraiser Form - Any team that wants to participate in team fundraising activities must fill out this form
Team Sponsor Form - Any team that wants to solicit individuals/businesses to sponsor their team must fill out this form

Travel Team Uniform Sizing - Use this form to type in names, numbers and sizes for uniform sizing.

Travel Team Registration Packet Cover Sheet - Use this form to neatly type all registration information for the team.
